BEFORE YOUR VISIT
We are proud to utilize technology in order to make your visit more efficient! We encourage enrollment in our patient portal as all medical information, communications, and office business will be conducted through the portal. To save time during check-in, please enroll and update your information before your visit. We can gladly help you enroll during your visit as well if you prefer.
Patient Portal Enrollment Process:
- You will receive a link by email
- Click on the link and follow instructions to create a user name/password
- Update any medical and insurance information

We will email the Patient Forms to fill out before your visit.

We will also email a copy of our Office and Financial Policies. You can also review our Office Policies.

FIRST VISIT
Please arrive 20 minutes before your scheduled appointment time to make sure all paperwork and insurance information is correct. This will also help us answer your questions and resolve any outstanding issues.

Check-in will be done and we can help you enroll in our patient portal, if needed.

Referral (if needed)

Prior imaging (CT, MRI, X-rays). Please have the discs with actual images for review in addition to any reports.

Prior office notes if seen by another ENT physician
